If you are coming in to our office for a Passport please

be to our office no later than 4:00.

Tips for filling out the forms:

  • Use BLACK INK (you will have to resubmit if you don't use black)
  • Do not use white out, if you make a mistake just cross out and rewrite.
  • DO NOT SIGN (we have to witness all signatures
  • All applicants need to appear in person unless you are renewing by mail.

What to bring with you (All applicants need to be present to apply):

  • Certified Copy of Birth Certificate
  • Valid Drivers License or Military ID
  • Prior Passport
  • 1 Recent Passport Photo
  • Fee
  • 2 Checks for the fees (One check will go to the Department of State and One will go to the Morton County Recorder)
